Guidance for Hospitalization

Before Your Hospitalization

You will be advised over phone upon availability of the bed.

Items to be Prepared at the at the Time of Hospitalization

Bedclothes are available from us. Prepare following items.
  Personal Seal
  Writing Materials
  Various Insurance Proofs (medical treatment proof)
  Copy of Family Resister or Residence Certificate
  Consultation Ticket Card (plastic Card)
  Admission Certificate and Interview Sheet for Hospitalization
  Consent Form for Hospitalization
  Medicine taking it now
  Undergarments, Pajamas, Dressing Gowns or Toilet Set
  Towel, Bath Towel, and Tissue Paper
  Chopsticks, Spoon, and Cup
  Footwear
We rent pajamas and gowns as a fee based service.

Procedures on the First Day of Hospitalization

1. Go to outpatients' reception desk at the notified time and date.
2. At the time of hospitalization, you are requested to receive necessary explanation at the outpatients' examination, then submit hospitalization certificate and relevant insurance card at the 2nd medical affairs department.
3. Nurse will guide you to the ward.
4. Submit one copy of family register, patient registeration card, various resevation cards and other requested documents to the nurse station of the ward.

Introductin of Your Days During Hospitalization

You will be supplied with the detailed instruction sheet at the ward. Following are the subjects that requires your special attention.
1. You are strictly requested not to bring in large amount of cash and valuables.
2. To bring in and use the handy phone is not allowed as it may cause mal-function of medical equipments.
3. Each bed is furnished with a TV sets (need pre-paid card, ear-plug).
4. As for the use of electrical items, consult with nurse leader at the ward before use.
5. Use coin-laundries for your personal robes and clothes.
