Request for the First Appointment

Schedule an Appointment

Cancer Institute Hospital requires a scheduled appointment for all the patients. Patients from abroad are required to fill out and submit the application form. Your request will be forwarded to the most suitable doctor from our organ-specific clinical departments taking the patient's medical condition into account.
Once your application form is processed, you will receive reply from International Patient Service Team (IPST) via an email or a call.

To make your first appointment, please apply online or fill out an application form and send it via Email/Fax/Mail to IPST with the following items.

Please attach the following items:

  • Referral letter from your current doctor – Please provide a referral letter in ENGLISH or JAPANESE.
  • Diagnostic Images or Diagnostic Imaging data (MRI, CT, Ultrasound, X-ray films, etc.) – The results based on which you have been diagnosed with cancer or your doctor has seen something suspicious.
  • Your previous medical records including the latest exam reports (If available)

Planning Your Visit from Overseas

Please bring:

  • Additional medical records (films, medical reports, if available)
  • All the medicines you currently take and their information
  • Information about your family's medical and health history - First time patients will be required to fill out the questionnaire at your registration.
  • Valid Visa and passport - Visa for Medical Stay is highly recommended to avoid having the patient go apply for the visa extension during the stay in Japan.
    Medical Excellence JAPANNew Window, Consortium Secretariat organized by Ministry of Economy, Trade and Industry, will help your arrangement of medical travel to Japan.
  • Japanese Currency (Yen)

Planning Your Visit for Current Japanese Residents

Please bring:

  • Japanese National Health Insurance card (if covered), and other grant certificate of disability, chronic disease, financial aid by Japanese Government, if any.
  • Additional medical records, if any
  • Information about the medicines you currently take, your family's medical and health history – First time patients will be required to fill out the questionnaire at your registration.
  • Japanese Currency (Yen)
