
Application Form for Clinical Observation

Before submitting the application, please carefully read and understand the terms and conditions for acceptance a clinical observation at the Cancer Institute Hospital of Japanese Foundation for Cancer Research.
Introduction.
- Depending on the time of year, we may not be able to provide a clinical observation or may ask you to reschedule your clinical observation.
- As a general rule, clinical observers are accepted for a maximum of three months.
- You will need to make your own arrangements for accommodations during the clinical observation period.
- Please note that the following observership fees are required for participation in our program.
Physicians: 5,000 JPY per day
Medical Students: 1,000 JPY per day
Medical Students: 1,000 JPY per day
- Upon completion of the observership, our hospital will issue a certificate of completion in our standard format.
If you require any documents other than our standard certificate (such as evaluation forms), an administrative fee of 2,000 JPY will be charged. (This does not apply to medical students.) - If you wish to request such documents, please inform us at the time of your observership application.
Conditions for Acceptance
- To have Ability to communicate in English.
- Must be able to submit documents specified by the Cancer Institute Hospital of Japanese Foundation for Cancer Research. (Certificate of employment, letter of recommendation from supervisor, etc.)
- Must have a visa to stay in Japan for a sufficient period of time
